Dear Parents/Guardians,
Our school needs you to complete this online survey to learn more about your opinions. It is an opportunity to communicate your perceptions about our school. Your participation is very important. Your responses will help guide our district and school efforts to promote safety, enhance learning supports, and improve student achievement.
• This is an anonymous survey. It is designed so that no one can be identified from the data.
• All questions apply to this school only.
INSTRUCTIONS FOR ACCESSING THE SURVEY
1. If you have more than one child at this school, you will only need to complete one survey for this school.
2. Please fill out just one survey about the school using your oldest child as a reference.
3. If you have children at different schools, you must complete a survey for each school.
4. After the first page of the survey, you will be able to change the language to one of 18 options.
